CORRELATION OF PLASMA INTERLEUKIN 1 LEVELS WITH DISEASE ACTIVITY IN RHEUMATOID ARTHRITIS WITH AND WITHOUT OZONE.

The mean plasma level of interleukin 1 beta was significantly higher in 51 patients with rheumatoid arthritis (RA) than in 21 rheumatoid arthritis patients treated with ozone, as control group of similar age in both groups.

 

Plasma IL-1 beta correlated positively with Ritchie joint index, pain score, and erythrocyte sedimentation rate and correlated negatively with haemoglobin concentration. These results support the idea that IL-1 beta has a central role in the phatogenesis of RA and the ozone has influenced in the concentration level of IL-1 beta.

IMMUNOLOGICAL FINDINGS IN THE PERIPHERAL BLOOD AND IN THE SYNOVIAL FLUID AFTER INTRAARTICULAR O3 OZONE INJECTIONS IN RHEUMATOID ARTHRITIS.

The aim of this pilot study is to find the influence that 02/03 exerts in the cytokines in peripheral blood and in synovial fluid. In this study we have two groups of classical rheumatoid arthritis. One group treated with 02/03 (major haemotherapy and intraarticular 02/03 injections), the other group without 02/03. We find there are significant changes with 02/03 (granulocytes and other cell population).

CLINICAL ASPECT OF 02/03 IN RHEUMATIC DISEASES.

Among the disorders collectively named as rheumatism, progressive chronic polyarthritis constitutes a great problem, not only as regards to the frequency of its occurrence (1-2 % of the population), but also as regards its most effective therapy.

 

Apart from non-medication (or physical) therapy methods, long-term antirheumatics, or so called basis therapeutics, represent the only medicaments known to have effects on the pathological processes involved, going beyond a symptomatic influence, contrary to the non-steroid antirheumatics and the corticoids.

 

In medical ozone research over the recent year, we have arrived at results with confirm the necessity for therapeutical application of an ozone-oxygene gas mixture, not only clinically, but also biochemically.

OUR EXPERIENCES IN THE USE OF O3 OZONE THERAPY IN THE ELDERLY.


During five years (1991-1995) we have used ozone therapy in the treatment of different diseases that are present in the elderly, for example upper and lower respiratory infections, arthrosis, pneumonia, immunodeficiency.

 

A statistical analysis showed improvement in more than 80 % of the treated patients. No side-effects or intolerance was present among the patients treated with ozone. It is concluded that ozone therapy is an important therapeutic option to be considered in geriatric patients.

O3 OZONE THERAPY IN THE TREATMENT OF OSTEOARTHRITIS.


We have been using ozone therapy, during 5 years, in 200 patients with different types of arthritis, for example: lumbar arthritis, generalized arthrosis, hiposteoarthritis, knee arthrosis.

 

In our group of patients, 60 % female and 40 % of male were present (80 % with more than 40 years old). Three groups of treatment were considered: group I , using ozone therapy alone; group II, ozone therapy plus medication (antinflamatory and/or analgesics drugs) and group III, ozone therapy plus medication and physiotherapy. In general. 85 % of improvement was obtained.

O3 OZONE THERAPY IN REACTIVE ARTHRITIS.


We have used ozone therapy, like part of the treatment, of 30 patients suffering from reactive arthritis, in the last three years. No significant difference in sex distribution were present, with predominance in ages less than 45 years. Patients with reactive polyarthritis and Reiter Syndrome were included in our study.

 

In 55 % of patients, reactive arthritis was consequence of upper respiratory sepsis. The clinical evaluation was satisfactory, with a fast improvement and without side-effects.

O3 OZONE THERAPY IN THE TREATMENT OF BONE INFECTIONS.


It was done an investigation about the therapeutic effects of ozone therapy in the treatment of bone and articular infections, in patients suffering of these lesions, admitted to the Service of Orthopedic and Traumatology of the General Teaching Hospital "Comandante Pinares", San Cristobal, Pinar del Río.

 

General treatment was performed using ozone rectal application and local ozone treatment and ozonized oil (OLEOZON®). Ozone therapy directly into the septic bone was applied as a unique treatment or combined with antibioticotherapy, surgery of lesions, etc.

 

Good results were obtained in a great percent of patients, but an outstanding improvement was seen in patients treated with ozone as the only therapeutic method. It was well accepted by patients and no side effects were observed. It was concluded recommending its application in the treatment of chronic infections of the osteomioarticular system.

OZ-PO-047: Inhibition of Interleukin-8 Synthesis intraarticular BY OXYGEN / OZONE O3 THERAPY IN PATIENTS WITH rheumatoid arthritis.

5 Patients with definite rheumatoid arthritis and knee effusions under constant doses of therapy NSARD Were Treated with up to 6 intraarticular (ia) injections every 3 to 7 days. Oxygen / ozone randomized matched controls who received a single group of 40 mg triamcinolone iainjection hexatonide (CT) Was Monitored According To the Same protocol.

 

The intraarticular granulocyte counts and IL-8 Levels decreases in oxygen / ozone Patients Treated on day 10-13 and stayes low in Those Patients Who could be re-Evaluated after 13 weeks. Compared to the IL-8 Levels, the other tested cytokine Levels Showed only minor changes file on day 10-13. There Was no need for a re-injection in the TC group DURING 13 weeks study phase. 

 

We Conclude That intraarticular oxygen / ozone therapy results in a DECREASE of SF-granulocyte counts. This effect may be due to the impairment of IL-8 mediated chemotaxis by IL-8 synthesis Decreased in synovial fluid mononuclear cells. 

 

Clinically, Repeated intraarticular oxygen / ozone therapy results in a worse outcome than iasteroid 13 week Treatment Measured in an intention-to-treat analysis.

OZ-PO-048: OZONE IN THE TREATMENT OF CHRONIC OSTEOMYELITIS.

Chronic osteomyelitis with fistula symptomatic secreting courtship, pyoderma pyogenic and pocket formation, is a condition of surgical solution. However, this, phenomena associated with sepsis, bone sclerosis and edema, cause a local decrease of oxygen that alters the tissue response against infection. 

 

That's when ozone acts as an adjuvant for its action phenomenon of biological oxygenation, stimulation of defense systems, increased tissue oxygen and also its local germicidal action, either as gas or by reacting it with vegetable oils. 

 

Based on the above ozone therapy was applied, along with common surgical procedures in 30 patients (Group A) and 10 patients (Group B), when procederes had been completed and wanted to prevent reactivation of the process. Ozone was applied by intramuscular injection, by autohemotherapy greater and local treatment (in bags and sub-atmospherically), along with ozonized oil cures. 

 

In Group A, 76% of patients achieved excellent and good results from, the suspended drainage, adequate skin coverage available and create a space suitable for bone grafts or other procedures within 60 days of testing. 

 

In group B was prevented recurrence after 30 d of application, 80% of patients.  Our results give a high value of ozone therapy in the treatment of sepsis of the members.

A SOLUTION FOR POST-TRAUMATIC OSTEAL EXPOSITIONS: ASSOCIATION OF GREATER OMENTUM GRAFT WITH O3 OZONE THERAPY.

 

Due to the frequency of injuries with postraumatic osteal exposition of the limbs, it is reported the use of greater omentum in association with ozone therapy, as a practical solution to correct these defects, with exposition of the fracture focus and in the presence or not of local infection.

 

It is demonstrated that with the association of these methods, healing time may be shortened, due to the early appearance of granulation tissue, even on osteal surfaces and a decrease in the inflammatory signs in a septic medium, with no signs of biological rejection.

 

Also, wasting material, anesthetics, drugs, and stay length may be saved, taking the graft from patients operated of puerperal ligatures, previous acceptation of donors and recipient. The graft implantation is performed with local anesthesia and there is no previous preparation of the injured patient. It is fixed with alternative sutures, its cost is very low and the patients feels highly satisfied.

 

It is concluded that the association of greater omentum graft with ozone therapy offers a good alternative therapeutic in the treatment of this disease.

USE OF OZONE THERAPY IN REITER'S SYNDROME.


Reiter's syndrome is a clinic tetrad of unknown etiology, consisting of urethritis, arthritis, mucocutaneous and conjuntivitis. It can be consecutive of an infection by chalamydia, campylobacter, salmonella or yersinia, accompanied with a systemic reaction that include fever. Occurs manily in young men and recurrences are very common.

 

Ozone therapy was applied in 10 out-patients of Dr. Octavio de la Concepción y de la Pedraja Hospital, in Camagüey, from 1995 to 1996, suffering from this disease. A descriptive method was used in this study. It comprised 10 male patients; the age-group that prevailed was among 20-25 years old.

 

The positive evolution of conjuntivitis was from the 3rd session on of ozone by rectal application.

 

In arthritis, 90 % had improved after the 5th session; as well as the other symptomp, showing the effectiveness as compared with the conventional treatment. There was not recurrence.

OZ-P-115: OSSEOUS RESIDUAL TREATMENT IN PATIENTS WITH CHRONIC OSTEOMYELITIS OF LONG TUBULAR BONES USING O3 OZONE.

We have treated 63 patients with lchronic osteomyelitis of long tubular bones. The purulent process was located on tibia - in 34 patients, on hip - in 29 patients. Posttraumatic osteomyelitis was diagnosed in 44 patients, haematogenous - in 14, gunshot - in 1, purulent complicated total hip joint arthroplasty - in 4.

 

The surgical strategy consist of sequestra and implants removal, osteonecrosis resection, irrigation and external fixation by Ilizarov device or plaster bandage. Immediately after the patient awakened, suction drainage of residual osseous and soft tissues cavity by saline solution with ozone concentration of 2,0 - 3,0 mg/L was started. On the 2nd–3rd day the perfusate was cleared of blood, on the 7th day fibrin was not seen. By the end of the second week the solution and the perfusate did not differ visually.

 

Average terms of drainage tube removal in control group (39 patients), were 24 days, in the sample - 17 days. Thus, using this procedure, in patients with purulent complications of lower limbs allowed to reduce the time of postoperative wound lavage and complete debridement by putting a suture. Local inflammatory reaction to drainage tube long-term stay in soft tissues was never found.

O3 Ozone Therapy and Acupuncture in degenerative Osteoarthritis. An Alternative Treatment.

In order to study the efficacy in pain treatment, as well as other signs and unpleasant symptoms in the degenerative osteoarthritis, two alternative therapeutic methods: ozone therapy and acupuncture were applied. A sample of 200 patients were divided at random, into two groups of 100 cases for each treatment (ozone and acupuncture). It was used an OZOMED equipment (Cuba) for the production of ozone.

 

Ozone group: patients with generalized arthritis were treated daily, by rectal application, with an ozone concentration of 50 mg/L and a volume of 200 mL. In patients with knee pains, ozone was injected in the antero-external part of the knee, using an ozone concentration of 35 mg/L and a volume of 20 mL, twice per week. In patients with shoulder pains, an ozone concentration of 35 mg/L with a volume of 10 mL was applied in the superior region of the shoulder, near the acromion, twice per week.

 

Acupuncture group: acupuncture was applied, in the pain area, three times per week, taking into account the theory of points and meridians, described by the Asiatic school. The needle was maintained during 20 min, with a manual stimulation each 5 min. In both treatments, the number of sessions were in relation with the pain disappearance and the improvement in the objective and subjective symptoms referred by the patients.

 

Ozone therapy and acupuncture achieved improvements of 59 % and 53 %, respectively, without significant differences between both groups. It was eliminated or diminished the number of medicines given to the patients, as well as those already described undesirable effects of the AINES, already mentioned in the world bibliography.

 

A significant economic saving is also reported in comparison with the traditional methods (analgesics, anti-inflammatory drugs). The employment of these methods allowed the patients to increase the independence in its daily works.

 

Adverse effects were not presented during and after these therapies. It was only need a scarce number of applications (between 3 and 7) in order to obtain an improvement, as well as the decrease or disappearance of the pains and of other symptoms and signs presented in this pathology. An increase in the quality of life of these patients was achieved.

Effect of O3 Ozone Therapy on the Level of Biomines in Patients with Rheumatic Arthritis.

The purpose of this article is to study the efficiency of ozone therapy in patients with rheumatic arthritis (RA), and first of all its effect on the dynamics of histamine, serotonin and catecholamine levels. Clinical observations in 140 patients with RA, at the age of 18 - 56 have been carried out.

 

In 70 patients (control group) were administered intravenous infusions of ozonated saline solution (0,9 % NaCl), at an ozone concentration of 2 mg/L in conjunction with the conventional therapy. The other 70 patients (the comparative group) did not receive ozone therapy.

 

Before and after the treatment, smears were taken from all the patients - imprints of periferal blood. To reveal catecholamines, serotonin and histamines fluorescence, histochemical technique with cytofluorometry in a fluorescence microscope, was used.

 

Intensity of luminescence was measured in conditional units. Original blood tests of the RA patients showed elevated figures of histamine, serotonin, and catecholamines in erythrocytes, polymorphonuclear leucocytes, lymphocytes, thrombocytes, and plasma.

 

In patients of the control group general condition after ozone therapy has improved, positive dynamics of the main parameters of rheumatoid process activity has been noted: ESR (erythrocyte sedimentation rate), levels of C-reactive protein, a2 - globulins, seromucoid, fibrinogen, hydroxiproline (p = 0.01), as well as albumine globulin index (p = 0.01).

 

The reduction in the medication dosage for the patients of this group started from the 5 - 6th day. Dosages of nonsteroid preparations were cut on the average by 30 - 40 %. In half of the patients taking glucocorticoids, medications were withdrawn, in the others the intake reduced by 30 - 40 %.

 

During ozone therapy more rapid regression of systemic manifestations has been observed (vasculitis, peripheral lymphodenopathy).

 

The most significant changes of biogenic amine levels have been noted in polymorphonuclear leukocytes and lymphocytes.

 

The reduction of all biogenic amines during ozone therapy was significantly more marked in the control group compared to the patients undergoing conventional therapy.

 

Thus, positive effect of ozone on one of the pathological link of rheumatic arthritis has been clearly demonstrated.

Knee and Ankle Arthrosis Treated with O3 Ozone Therapy

The aim of this work is to demonstrate the efficacy of intra-articular ozone therapy in different patients suffering of knee and ankle pathologies. The results and discussion of cases are presented in videos. It was used ozone by intra-articular administration, at a concentration between 20-30 mg/L and a volume between 7-10 mL, 3 times per week, during 1 or 2 months.

 

Three patients suffering from knee arthrosis (patient of 65 years old suffering of genu varum, young patient with condylar degeneration and a patient with knee infection) and 3 suffering from ankle arthrosis (patient with peroneal tibialis astragalus arthrosis, young sportman patient with malleolar lesion and adult female with ankle arthrosis) were treated. Good results were achieved, demonstrated by X-ray and clinical evidences.
